Currently I am accepting Anthem, Aetna, Cigna, Optum, UHC, UMR, OhioHealthy, OSU Health Plan, Medical Mutual, Lyra, Tricare East, Medicare, Buckeye, and Caresource Medicaid and Market Place insurance. I also accept self-pay clients. Most insurance plans cover online counseling. All patients must be Ohio residents. All major credit cards, cash, or checks are accepted and payable at the time of scheduling your appointment. In person, phone, and video counseling offered. Sliding Scale Fee available based on need.
Fee Schedule:
$150 for initial intake session
$120 per 60 minute individual office session
$120 per 60 minute online counseling session
$120 per 60 minute telephone counseling session
$100 per requested report
$30 bounced check fee
$50 per hour for supervision
